What if you trimmed the main section back a bit so the offshoot acted like a hilt?

Good idea, but the overall length is to short for that.
I have wanted to do that , but I have never found an antler that had the right configuration.
It gets frustrating sometimes, I'll have a very neat antler with some brow spikes but I have to cut them off because they will be in the palm of your hand while holding the knife.
But on the flipside, I have had the brow spikes work out to be finger grooves.

Very nice.

I got a couple small antlers I want to send but one is missing the base. I don't know if they are worth messing with?

There is options. Could make a wood or brass pommel.
If all else fails, pull knobs and buttons.
